Online Form 10s
For every new member, or for change of status and address, a Form 10 needs to be filled out. Do so here!
Online Form 11s
This needs to be filled out after every election of officers.
Dispensation Request
Fill out this form to obtain special permission from the Executive Officer.
Driving & Insurance Policies
Everyone must adhere to the Michigan DeMolay Driving Policy. Health Information is also required to be on file for every active DeMolay and must be carried by an advisor while traveling. Get the Health Information form here!
Youth Protection
Nothing is more important than the safety of our youth, and DeMolay International has developed a DVD that must be shown at least twice a year. It is comprised of small discussion modules and should be shown at least every other month. Click here for the Proof of Attendance form, which needs to be filled out and filed with Michigan DeMolay twice a year before each installation.
Yearbooks & Fundraising
Michigan DeMolay produces an annual Yearbook that commemorates the past year. This book provides funds to the Chapters and Michigan DeMolay through the sale of the advertisements which appear in the yearbook. Hello everyone, I hope you all had a great time this month as a lot of exciting events in February. We had Davison’s State Wide Rally, and the Heartland Qualifier which both turned out to be great events. We also had a great week of degree work throughout the state where we welcomed nine new brothers into our order! I hope everyone is looking forward to the installation season and I look forward to seeing you all in my travels.
